Documents Required for Medical Cannabis Card in Arizona

The ADHS application must be completed online, and you will need additional documents to complete the application process once you have a recommendation letter from the licensed mental healthcare professional.

  • Arizona Driving license

  • A current photo (not more than 60 days old)
  • A completed and signed qualifying patient attestation form

How To Get a Caregiver License in Arizona

The role of MMJ carers is growing significantly as more patients use cannabis as part of their medical treatment across the state. A trusted individual who helps a patient with marijuana treatment is known as a carer. The patient is assisted by carers while they smoke, consume, vape, or apply oils. Having support from others can be quite helpful since many MMJ patients are unaware of or new to the marijuana treatment.

An Arizonan may be the designated carer for a maximum of five patients, per the ADHS. A carer must log into their ADHS account and list every patient they intend to assist. Prepare the necessary materials before submitting an Arizona carer application.

  • A digital photograph (no more than 60 days before application)
  • A scanned copy of the driver’s license, state identification card, or U.S. passport
  • A completed medical marijuana caregiver attestation form
  • Fingerprints and a fully completed fingerprint verification form.

Requirements

  • At least 21 years old
  • Registered with the Department of Health Services
  • Has not been convicted of an excluded felony offense
